Preparation for your appointment

After scheduling an appointment, you may download our patient forms by clicking on the button below. This way, you may fill them out in the convenience of your home. Just bring them with you for your first appointment. The information will then be reviewed and discussed with you.

 

What you can expect at your Initial Evaluation:

  • A thorough medical history and musculoskeletal assessment.

  • Internal pelvic floor exam (vaginal/rectal).

  • Pelvic floor biofeedback session.

  • An review of initial evaluation findings and opinion about your potential for improvement and meeting your treatment goals in pelvic floor therapy.

  • Setting a series of appointments to treat your condition and guide your progress.

  • An assigned specific recommended home program which requires your active participation.

 

Treatment:

Treatment is based on the findings during your initial evaluation. Typical treatment in the clinic includes manual therapy, exercise, biofeedback, perineal electrical stimulation, exercise on state-of-the-art Pilates Reformer and Core Align, behavioral and diet modifications, and postural changes. It is important to understand that treatment requires time and devotion by both the physical therapist and patient. It is not a quick one-time-only treatment.

 

Referrals and Insurance:

In Nevada, patients have direct access to be evaluated and treated by a physical therapist without a physician's referral; however, problems with bowel, bladder, or pelvic pain should be evaluated first by your medical doctor. Also, your insurance guidelines and policy may require a doctor's prescription for reimbursement. If you come to therapy without a medical referral, we may request you visit your healthcare provider if the therapist feels it is in your best interest.

 

Back In Motion Physical Therapy is contracted with the following insurance companies:

  • Aetna

  • CDS Group Health

  • First Health

  • GEHA

  • Hometown Health Plan

  • Medicare

  • Prominence

  • Senior Care Plus

  • Tricare For Life

  • Tricare West

 

If we are not contracted with your insurance provider, you may either 1) request a Gap Exception from your insurance carrier since our practice provides a highly specialized service not available in your network or 2) you have a self-pay option.  In that case, we can provide you a receipt, the 'super bill', for self-submission.  It includes billing (CPT) code(s) and your diagnosis code. Any payment your insurance agrees to reimburse will be mailed directly to you.  Feel free to call us at (775)746-2206 and Sabine will help you through all the options.
